Part 1: APPLICATION AND SUBMISSION OF DOCUMENTS
Step 1: Prepare the document requirements
Philippine Statistics Authority (PSA) Birth Certificate
Current Report Card (must have a Learner Reference Number (LRN) from the previous school which is DepEd Recognized / Accredited School) —for incoming Grades 1 to 11 only.
Additional requirement for incoming Grades 8 to 10
Step 2: Submit document requirements.
Option1: Upload the scanned copy of documents to the Pre-application form or email them to firstname.lastname@example.org
Option 2: Submit printed/photocopy of all required documents in a sealed long brown envelope
clearly marked with the APPLICANT’S NAME. This may be dropped off at the Registrar’s Office.
Step 3: Wait for acknowledgment of receipt and processing of application documents.
Part 2: ENROLLMENT
After the evaluation of submitted forms and documents, enrollment follows.
The schedule of enrollment will be posted on the DBTI Talac Website and Social Media pages.
