Step 1: Prepare the document requirements

  1. Application Form

      • Option1: Fill out the online Pre-application form

  2. Philippine Statistics Authority (PSA) Birth Certificate

  3. Baptismal Certificate

  4. Current Report Card (must have a Learner Reference Number (LRN) from the previous school which is DepEd Recognized / Accredited School) —for incoming Grades 1 to 11 only.

  5. Additional requirement for incoming Grades 8 to 10

      • Recommendation Form duly-accomplished by Guidance Counselor/Class Adviser and Principal of the present school attended.

Step 2: Submit document requirements.

  • Option1: Upload the scanned copy of documents to the Pre-application form or email them to

  • Option 2: Submit printed/photocopy of all required documents in a sealed long brown envelope

clearly marked with the APPLICANT’S NAME. This may be dropped off at the Registrar’s Office.

Step 3: Wait for acknowledgment of receipt and processing of application documents.


After the evaluation of submitted forms and documents, enrollment follows.

The schedule of enrollment will be posted on the DBTI Talac Website and Social Media pages.

  1. Make sure to submit the complete requirements. Incomplete requirements will not be processed.

  2. The Final Report Card is to be submitted in hard copy before the enrollment for School Year 2022-2023.

  3. Incoming grade 1 applicants will undergo an online interview. After the evaluation of the submitted requirements, the parents of the applicants will receive an email with the specific date and time of the interview schedule.
