Part 1: APPLICATION AND SUBMISSION OF DOCUMENTS
Step 1: Prepare the document requirements
Application Form
Option 1: Secure an application form from the Office of the Registrar.
Option 2: Download the Application Form.
Clear photocopy of Philippine Statistics Authority (PSA) Birth Certificate.
Clear photocopy of Baptismal Certificate.
Clear photocopy of Current Report Card (must have a Learner Reference Number [LRN] from the previous school which is DepEd Recognized / Accredited School - for incoming Grades 1 to 11 only)
Three copies of most recent 1x1 colored pictures with white background.
Additional requirement for incoming Grades 8 to 10.
DBTI Recommendation Form duly accomplished by Guidance Counselor/ Class Adviser and Principal of the present school attended.
Step 2: Submit document requirements to the Registrar's Office.
Step 3: Pay the Admission and Testing Fee of Php 300.00 to the Cashier’s Office and secure a Test Permit from the Registrar's Office.
NOTE: Make sure to submit the complete requirements. Incomplete requirements will not be processed.
Part 2: ADMISSION TEST
Step 1: Take the Admission test administered by the Guidance and Testing Office (GTO) on the following schedule:
For Nursery to Grade 3 Pupils: Every Mondays and Tuesdays (March to May 2025) with the following timeslots - 9:00 AM to 10:00 AM, 10:30 AM to 11:30 AM, 1:30 PM to 2:30 PM, and 3:00 PM to 4:00 PM; and,
For Grade 4 to Grade 11: Every Fridays (March to May 2025) from 4:15PM to 5:15 PM.
Step 2: Familiarize yourself with the GTO Testing Rules and Procedures.
Step 3: Test results will be sent via email to the parents of the applicant approximately one (1) week after the exam for Grade 4 to Grade 12. For Nursery to Grade 3 applicants, it will be communicated and discussed with the parent/guardian right after the assessment. No results will be released by telephone.
NOTE: Walk-in testing applicants will not be entertained.
