Dearest Parents,


Happy 24th of the Month!


We hope that you are well. It is timely that I am able to reach out to you on this day that we commemorate Mary Help of Christians. COVID-19 is still a prevailing threat but we are happy that our doctors and scientists work round the clock to provide us with vaccines against it.


We are happy to announce that, once again, the City Health Office of Tarlac City has partnered with Don Bosco Tarlac for the vaccination of our youngest Bosconians. This time we will welcome for pediatric vaccination our Bosconians aged 5 to 11 on March 4, 2022, Friday, from 8:00 AM to 12:00 PM in our campus.


If you want your Bosconian, aged 5 to 11, to be vaccinated please follow these easy steps:

  1. Register your interest using this online form until Tuesday, March 1, 2022.

  2. Prepare and be ready to bring the following documents for assessment:

    1. Proof of relationship

    2. Birth Certificate of the child

    3. Valid ID of the child with a picture

    4. Valid ID of parent/legal guardian with picture and Vaccination Card

    5. Authorization Letter duly signed by the parent, if accompanied by a guardian.

    6. Medical clearance for those students who have comorbidities

    7. Filled-out Health Declaration and Consent Form

      2. Print on Long Bond Paper, 4 pages.

  3. Wait for the time slot allotted for your Bosconian (https://www.dbtarlac.edu.ph/dbvax). If you have multiple Bosconians registered for the vaccination, all of them can be accommodated in one time slot.

  4. On March 4, 2022, please accompany your Bosconian to Don Bosco Tarlac in your designated time slot. Bring all necessary documents (See number 2 above ↑) Wear a well-fitted face mask, bring your own alcohol/sanitizer and writing materials. Observe the health protocols. Make sure that you and your child are not hungry and thirsty. If needed, bring your own bottle of water.
